Application Form Demo
This is an example of an application form for opening a new or
additional bank account as a private customer.
It's been written with a large German bank in mind so the form
field names below are also given in German (in parenthesis).
After submitting the data a PDF page will be generated on the
spot, containing your data entered in the form below as well as
some unique identifier, that you can download, and - if this was
a real bank originating it - sign and send it to your bank.
For brevity this form asks only for 'personal data' fields
to be filled in (in 'real life' there would be more fields).
